While emphasizing that personnel can decline treatment, the broad scope of the mandate has raised alarm bells within the medical community.

The Endocrine Society, a leading organization of hormone specialists, has issued a formal statement cautioning against such population-level screening. Over time, untreated hypogonadism can contribute to a loss of body hair, decreased bone density (osteoporosis), and a reduction in red blood cell production, potentially leading to anemia.
However, Anawalt highlights the significant challenge in diagnosing testosterone deficiency in the general population due to the vagueness of symptoms. Many men experience non-specific complaints such as fatigue, low energy, mild erectile issues, concentration difficulties, or general malaise. While these concerns are valid, they are not uniquely indicative of testosterone deficiency and can be attributed to numerous other factors. Attributing these vague symptoms solely to low testosterone, without thorough investigation, can lead to misdiagnosis.

The accuracy and reliability of testosterone testing itself present another significant hurdle. Anawalt emphasizes the critical need for validated laboratory assays. "Tests that measure testosterone are a disaster unless you use a CDC validated or certified testosterone assay," he warned. However, for individuals without such a clear medical need, the benefits of TRT are far less certain. Anawalt asserts that administering TRT to a man with already normal testosterone levels is unlikely to enhance performance. For a 50-year-old soldier with only slightly low testosterone, the substantial benefits of TRT are often not realized.
The evidence supporting Hegseth’s claims of enhanced performance and longevity through TRT in healthy individuals is limited. While TRT may restore natural longevity in those with diagnosed disease, there is no robust evidence to suggest it extends the lifespan of healthy individuals. Similarly, studies on the cognitive effects of TRT have yielded no measurable benefits.
Potential Harms and Side Effects of TRT
The risks associated with TRT are a significant concern, particularly for a mandated screening program targeting younger populations. Key risks include:
- Infertility: TRT suppresses natural sperm production, which can impair fertility. Recovery of sperm counts after discontinuing therapy can take months to over a year, with longer treatment durations and higher doses potentially leading to longer recovery times.

He predicts it will lead to a surge in "inappropriate evaluations, inappropriate diagnoses of testosterone deficiency, and likely going to lead to the prescription of testosterone to a number of young men."
There is also concern that this directive may foster a perception of testosterone as a "magic bullet," potentially encouraging unnecessary screenings and treatments not only within the military but also in the civilian population. Anawalt reports a six-month waiting list for patients seeking testosterone checks, and he has received inquiries from family members about initiating TRT without clear medical necessity.
The endocrine system is a complex network, and simplistic solutions can have unintended and detrimental consequences. Anawalt recalled a past hypothesis that testosterone could aid weight loss by boosting metabolism. While trials showed some weight loss, they also revealed declines in bone density, unfavorable shifts in muscle-to-fat ratio, and irregular heartbeats. This illustrates how intuitive or seemingly beneficial interventions can prove incorrect upon closer scientific scrutiny.
"We doctors, we’re not a cabal trying to withhold some secret recipe," Anawalt stated, emphasizing the scientific basis for caution. "If testosterone was good for all men, we’d be telling everybody that. We’re not withholding information. What we’re saying is that the information available and prior experience suggests we should be cautious and do studies."
The Pentagon’s move toward mandatory testosterone screening represents a significant divergence from established medical guidelines and expert consensus. As the program rolls out, close monitoring, transparent data collection, and a willingness to heed expert medical advice will be crucial to ensure the health and well-being of service members, preventing what many experts fear could become a large-scale, medically questionable intervention.
